Comprehensive Diabetes Self-Management Training (DSMT)

Our LifeSkills Training Program teaches your patients the skills to take control of their diabetes in an easy-to-understand group setting.  Each session is about two hours long with afternoon and evening classes available.  The group sessions meet four times within a four to five month period.  Sessions are guided by a group of professionals that include a Registered Dietitian, Certified Diabetes Educator, Registered Nurse, and a Physical Therapist.  Each session focuses on a unique aspect of diabetes care.

Diabetes Individual Training

Individual education services are available for your patients to meet with a dietitian or certified diabetes educator to help guide them to a better understanding of either a new diagnoses of diabetes or uncontrolled diabetes. This individual training session will provide your patient with one-on-one comprehensive education and new skills to help manage their condition.  If the educator feels that your patient may need extended education they will be offered a follow up appointment or be enrolled in our Comprehensive Diabetes Self-Management Training (DSMT).  The educator will provide you with documentation of the session along with any recommendations.

Gestational Diabetes Training

Gestational Diabetes Management appointments are perfect for expectant mothers who want to learn how they can navigate diabetes treatment during pregnancy. Appointments will focus on nutrition counseling, glucose monitoring goals, and understanding how gestational diabetes can be controlled for the health of both mother and baby. The likelihood that women with gestational diabetes will develop type 2 diabetes later in life is three to sevenfold higher within a 5 to 10 year period. These appointments are a good start for prevention during this period.

Medical Nutrition Therapy Services

Our dietitians can help your patients with a variety of conditions learn the nutritional and meal planning skills they need to lead a healthier lifestyle. In addition to diabetes, we see people with kidney disease, cardiovascular conditions, gastro-intestinal issues, obesity and weight gain, unexpected weight loss, and others.

Diabetes Support Groups

DMC has quarterly diabetes support groups throughout the year that focuses on different issues of diabetes care.  This support group meets at different venues in the community.  Please contact 304.637.3343 for dates and more information.

Telediabetes

DMC is one of the first in the state of WV to offer telediabetes to rural clinics throughout the state.  The same services that we offer during a one-on-one consultation can also be provided by utilizing telehealth technology.  Please contact us for further information.

Contact and Referral Process

The referral process is easy and convenient. Download and complete the REFERRAL FORM and fax to the number provided below.  Your patient will be contacted concerning the appointment day, time and the cost of the service depending on insurance coverage.

Please note: Some health plans may not cover all of these services. Our scheduling department will contact their insurance company for authorization.
